For Example:
• Other programs/populations might be added to a particular procedure code. Pgm Ind column.
• A modifier might be added, or removed. M1 thru M4 columns.
• Program indicator Description might change . Pgm Ind Description column.
• Number of units allowed might have changed. Min Units / Max Units columns.
